(Psychosocial and behavioral predictors of outcomes pre- and post cardiac transplantation in the Waiting for a New Heart Study)

Durchgeführt in Kooperation mit Prof. Dr. Gerdi Weidner, San Francisco State University (San Francisco, CA, USA), Eurotransplant International Foundation (Leiden, NL) sowie 17 Kliniken in Deutschland und Österreich.
The Waiting for a New Heart Study, conducted in collaboration with Eurotransplant and 17 collaborating hospitals, is the first prospective, multi-site study to identify psychosocial and behavioral predictors of waiting list outcomes independent of medical risk. From April 2005 to December 2006 318 newly listed transplant candidates were enrolled, of whom 151 were transplanted and 62 were still waiting for a donor heart by December 2008. We showed that depressive symptoms, social isolation, eating habits, and gender contribute to waiting list outcomes independent of disease severity. As pre-transplant urgency and estimated post-transplant success denote the basic principles for organ allocation, the follow-up will be extended until December 2012. By then, all participants are expected to have reached one of the waiting list endpoints (death, transplantation, delisting due to clinical deterioration or improvement). Greater event numbers and an estimate of 200 transplanted patients will provide the unique opportunity to pursue two major aims: (1) to examine the joint contribution of psychosocial and behavioral risk factors for pre-transplant outcomes using a state-of-the-art competing risks approach, and (2) to investigate the role of psychosocial and behavioral characteristics for post-transplant short- and long-term survival, adjusting for disease severity and transplant-related variables.

Psychosoziale Faktoren bei Tako Tsubo-Kardiomyopathie
(Psychosocial factors in Tako Tsubo cardiomyopathy)
Durchgeführt in Kooperation mit der II. Medizinischen Klinik der Universitätsmedizin Mainz (Prof. Dr. Thomas Münzel)
Bei Tako-Tsubo-Kardiomyopathie, einer Differentialdiagnose des akuten Koronarsyndroms, wird akuter Stress als Auslöser der Symptome diskutiert. Unbekannt ist bislang, ob den Patienten andere psychologische Merkmale gemeinsam sind. Ziel dieses Projektes es, Patienten hinsichtlich auslösender Stressoren, der Zufriedenheit mit den Lebensumständen, psychischer Vorerkrankungen, Persönlichkeitsmerkmale und Mechanismen der Stressverarbeitung näher zu beschreiben.
